Managing prescriptions, therapy, and daily support can mean contacting several providers. As your healthcare needs grow more involved, the Medicaid plan you have determines your available benefits and how your care is managed.

A Medicaid Tailored Plan brings these needs together for eligible North Carolinians. Here’s how to check eligibility, regional assignment, and benefits.

1. What Is A Medicaid Tailored Plan?

A Medicaid Tailored Plan is a type of North Carolina Medicaid health plan for people who need more specialized support for mental health conditions, substance use disorders, or certain disabilities.

Launched on July 1, 2024, these plans bring regular medical care and specialized services under one health plan. This means members can receive help managing their physical healthcare alongside more complex support needs.

Tailored Plan vs. Standard Plan

Standard Plans cover routine healthcare and many behavioral health services. However, some people need more intensive treatment or specialized community support than Standard Plans provide.

Tailored Plans are designed for those needs, offering access to additional services and Tailored Care Management for eligible members.

Unlike Standard Plans, Tailored Plans are assigned based on the county that administers your Medicaid case. North Carolina currently has four regional Tailored Plan organizations.

2. Who Qualifies For An NC Medicaid Tailored Plan?

You may qualify for a Medicaid Tailored Plan if you’re enrolled in North Carolina Medicaid and have more complex support needs related to:

  • Serious mental illness or serious emotional disturbance.
  • Severe substance use disorder.
  • Intellectual or developmental disabilities (I/DD).
  • Traumatic brain injury (TBI).

However, having one of these conditions does not automatically qualify you. NC Medicaid also considers your treatment history and the level of support you need.

People receiving services through programs such as the NC Innovations Waiver, TBI Waiver, or Transitions to Community Living may be required to enroll in a Tailored Plan when applicable. Some Medicaid members remain in NC Medicaid Direct under separate enrollment rules.

If you’re unsure which arrangement applies, contact the NC Medicaid Enrollment Broker to review your eligibility.

4. What Services Does A Medicaid Tailored Plan Cover?

A Medicaid Tailored Plan covers regular healthcare while offering additional support for eligible members with more complex needs.

Medical, Pharmacy, and Behavioral Health Care

Members can receive covered doctor visits and prescription medicines alongside mental health or substance use treatment.

This combined coverage is especially useful when someone needs ongoing treatment from different providers. Rather than managing separate health plans, they can work with one plan to coordinate covered care.

I/DD, TBI, and Community-Based Supports

Eligible members may also receive specialized services that support daily living, such as assistance at home or help participating in community activities.

Certain services are available through the Medicaid 1915(i) benefit, which requires a separate assessment. These services are not offered through Standard Plans, although eligible members may access them through Tailored Plans, NC Medicaid Direct, or the EBCI Tribal Option.

Tailored Care Management offers another form of support. An eligible member can work with a care manager to coordinate appointments and connect with available services. This support is optional and comes at no additional cost.

Transportation to covered medical appointments may also be available when eligibility requirements are met.

Keep in mind that joining a Tailored Plan does not automatically qualify you for every specialized service or waiver.

5. How To Check Tailored Plan Eligibility And Enrollment

If you already receive NC Medicaid, you may not need to submit a new application. The state automatically reviews members for Tailored Plan eligibility.

Step 1. Check your Medicaid coverage: If you aren’t enrolled, apply for NC Medicaid. Existing members can check their current plan on their Medicaid card or enrollment letter.

Step 2. Review your eligibility notice: NC Medicaid may notify you if you qualify for a Tailored Plan. If you’re unsure about your status, call the Enrollment Broker at 833-870-5500.

Step 3. Confirm your regional plan: Your assigned Tailored Plan depends on the county that manages your Medicaid case. Check the official Tailored Plan directory to find the organization serving your county.

Step 4. Request a review if needed: If you believe you qualify but haven’t been assigned to a Tailored Plan, you or your provider can submit a Request to Move for an eligibility review.

Step 5. Check your providers: Before your new coverage begins, ask whether your current doctors and therapists participate in the assigned network. If not, contact the plan about possible continuity-of-care arrangements.

Confirm your effective date before making changes to existing appointments or treatment arrangements.

6. Complex Care Often Means More Calls: Lifeline May Help

Managing several healthcare appointments can mean frequent calls with your care manager or pharmacy. When you’re already dealing with ongoing medical needs, keeping phone service affordable matters.

If you receive Medicaid, you may also qualify for Lifeline, a federal program that lowers wireless communication costs.

Other ways to qualify include SNAP, SSI, Federal Public Housing Assistance, or household income at or below 135% of the Federal Poverty Guidelines. Lifeline requires one benefit allowed per household, and it is non-transferable.

Lifeline supports the discount, while an approved Eligible Telecommunications Carrier (ETC) provides service and applies the savings. Enrolling with a participating carrier connects your approved benefit to an active service plan.

Cintex Wireless offers Lifeline wireless service alongside free or discounted device promotions. The carrier has recently merged into AirTalk Wireless, expanding its existing device selection. During the merger, some applications may redirect to AirTalk’s website.

Depending on your ZIP code and current inventory, phone options to explore include:

Compare device prices, service coverage, and monthly data allowances before applying. Promotions depend on eligibility, availability, and offer terms. Choosing a suitable phone and plan can make it easier to return care-manager calls, confirm rides, and keep follow-up appointments organized.

IMPORTANT: The government does not subsidize devices. Lifeline programs cover basic service costs only. Free or discounted devices, upgrade plans, or top-ups are exclusive benefits provided by AirTalk Wireless and Cintex Wireless as part of the promotional offers. Terms and conditions apply. Limited-time promotion—offers vary by state, stock availability, and eligibility.

7. FAQs

Is a Tailored Plan available outside North Carolina?

No. NC Medicaid Tailored Plans are specific to North Carolina. Other states may offer similar specialized Medicaid programs, but their benefits and enrollment requirements differ.

Does a mental-health diagnosis automatically qualify me?

No. A diagnosis alone does not guarantee Tailored Plan eligibility. NC Medicaid also considers your treatment history and support needs. Contact the Enrollment Broker if you believe you qualify but haven’t been assigned.

Can I keep my current doctors and therapists?

Possibly. Check whether your providers accept your assigned Tailored Plan. If they’re out of network, contact the plan about continuity-of-care protections or possible exceptions before scheduling future visits.

Does enrollment guarantee NC Innovations Waiver services?

No. Tailored Plan enrollment does not automatically qualify you for NC Innovations Waiver services. You must meet separate waiver requirements, and a slot must be available.

Conclusion

A Medicaid Tailored Plan offers eligible North Carolinians access to specialized healthcare and support beyond what Standard Plans provide. However, eligibility for the plan doesn’t automatically guarantee every service.

If you think a Tailored Plan may fit your needs, confirm your eligibility and regional assignment with NC Medicaid.
